Introduction :
In PMH Community Nursing Service (CNS), there are approximately 40,000 outreach nursing visits and 5500 new referrals every year, in which around 10% are indicated for drug management requiring drug preparation and empowerment by community nurses. Community nurse plays a significant role in ensuring safe and effective use of drugs at patient’s home. During the first visit, community nurse executes drug reconciliation, conducts comprehensive assessment and formulates a care plan on patient’s drug management. To ensure service quality and standardized workflow under growing service demand, an innovative e-learning tool - audiovisual training material on Patient Support on Drug Management in PMH CNS was produced to highlight the gist of daily practice and provide up-to-date information to staff.

Methodology :
The contents of the video were evolved from the department training on Drug Management in the Community in April 2022 with principles grounded on HA Basic Nursing Standard for Medication Management - Oral. The video illustrates the gist of practice and patient/ carer empowerment in drug management throughout the whole patient journey from hospital to the community in 4 main phases: 1) Pre-discharge assessment, 2) Preparation before home visit, 3) During home visit and 4) Preparing the dosette. Important messages and practical tips, such as checking inpatient drug administration record and clarification when in doubt, are highlighted in the video to ensure seamless handover from hospital setting to community setting. Use of HA apps is introduced to facilitate the whole drug management process in the community.
Result & Outcome :
The video was first run in PMH CNS Staff Meeting in November 2022 and was uploaded to department webpage, allowing staff to review anytime and anywhere on need basis. The video had become part of training in CNS Preceptorship Program for new comers. Positive feedback on the video was received from all staff. Staff compliance to drug management and administration would be assessed yearly by supervisors to ensure service quality and outcomes. For betterment of community care development, this e-learning tool in community was promulgated and shared with other hospitals in the Kowloon West Cluster for training and standardized practice in drug management.
