Introduction :
Pharmacists at DKCH Pharmacy runs a Counselling Clinic for Same Day Admission (SDA) of Joint Surgery Patients since 24/07/2020.
The counselling clinic conduct in parallel with anesthetist at PAC (pre-admission clinic) once per week on Friday afternoon. The patient journey of the day may include visiting to the Physiotherapist, Occupational Therapist, X-ray, Anesthetist, Pharmacist and Nursing.
Objectives :
At this PAC, Anesthetist conducts final assessment, complete consent with patient and decide on the suitability of patient admission for day case before the actual scheduling of surgery. Pharmacist review each patient's medication profile, provide advice that tailors to their use of medications before surgery. The concerned class of medications included anticoagulants, hypoglycaemic agents, pain medications, anti-rheumatic drugs were discussed at pharmacist clinic session.
Methodology :
A 2 years’ review is conducted and interesting cases are highlighted for sharing.
Result & Outcome :
Over the course of 2 years, value-added services provided by clinical pharmacist at outpatient may include but not limited to i) the review of patient's medication compliance, ii) re-assuring the importance of home blood pressure and blood glucose monitoring, iii) discussion on side effect of medications, iv) advice on the use of supplements and traditional chinese medicine before surgery, v) provision of counselling material in relation to their medication profile to joint operation.
