Enhancement of drug delivery workflow from QEH Medical Wards to Convalescent Hospitals by “All-in-One” delivery chop and “One-page Summary”

Introduction :
Queen Elizabeth Hospital is an acute hospital. Medical patients would be transferred to convalescent hospitals including Kowloon Hospital (KH), Buddhist Hospital (BH), Wong Tai Sin Hospital (WTSH) and Our Lady of Maryknoll Hospital (OLMH) for extended care. Physicians would prepare three to five days’ drug prescriptions via Medication Order Entry (MOE) and ward staff would send the MOE to pharmacy department for continuing medication supply in convalescent hospitals. Staff need to use different drug delivery chops to stamp on MOEs and fax to QEH pharmacy or receiving wards depending on transfer time and designated hospitals. Currently, there are three different drug delivery chops used in medical wards, including “Transfer to KH/BH”, “To WTSH/OLMH” and “Discharged by NEATS”. Ward staff have to choose one of the appropriate drug delivery chops when used. It was confusing and clumsy to ward staff, especially for newcomers.
Objectives :
To assess the acceptability of “One-page summary” and “All-in-One” drug delivery chop by Pharmacy staff and Ward staff in the Department of Medicine of Queen Elizabeth Hospital.
Methodology :
Task Force on Drug delivery workflow was formed under Department of Medicine. Factors for slow down drug delivery workflow were identified. “One-page summary” for drug delivery workflow and “All-in-One” drug delivery chop was designed. All drug delivery conditions were summarized in “One-page summary” and “All- in-One” drug delivery chop. Ward staff could handle the MOE accurately and effectively according to workflow instruction. The drug delivery workflow and new drug delivery chop were shared in Department meeting. Onsite briefing was provided by ward managers to frontline nursing and clerical staff. Staff satisfaction survey was conducted for their acceptability.
Result & Outcome :
Department nurses and clerical staff were invited to participate this survey. Total 101 staff completed the survey and over 89% staff highly agree to the enhancement of drug delivery workflow. There are more accurate instructions from “One-page summary” & “All-in-One” drug delivery chop. The new chop displayed concise information of drug delivery workflow & it can facilitate drug delivery in order to enhance effectiveness & accuracy. Besides, the new chop and workflow could facilitate the non-medical staff of contingency ward to have effective drug delivery during COVID-19 and winter surge. The enhancement of drug delivery workflow can improve effectiveness of drug delivery in acute ward setting & was well-accepted by ward & pharmacy staff.
