Introduction :
Paper documentation of Photoguide (MR02086) is well adopted in our department for complicated wound care clinical handover. To align communication and maintain the consistency in wound care as well as enhance the quality and safety of wound packing, a colour-printed photo guide is employed to assist nurses in the steps of changing dressings. Although all photos are protected in encrypted folder, there is barrier for privacy data storage. Thus, we make use of clinical album to take sequence of clinical photos with descriptions to sustain the concept of Photoguide under CMS.
Objectives :
Utilizing album potential to align communication and maintain the consistency in wound care as well as enhance the quality and safety of wound packing
Methodology :
Individualized photo-guides are made for all complicated wounds and when changing of new dressing methods or materials. Firstly, verbal consent is obtained from patients for wound photo taking before usual comprehensive wound assessment. Then photos are taken for every step of the dressing techniques. Next, nurses prepare the individualized photo-guide with clear descriptions on every step including wound condition description, disinfecting lotion used, dressing materials as well as number of packing removed and used. Lastly, nurses use the clinical album guide for handover. Nurses capture clinical photos when change dressing so that surgeons can closely monitoring wound healing progress.
Result & Outcome :
Implications: 1.Alleviate redundant clerical work 2.Promote wound healing by implementing the consistency of wound care process 3.Unify wound care practices, polishing wound dressing and packing technique 4.Increase staff confidence in encountering different wounds 5.Facilitate collaboration and transparency 6.Streamline daily work 7.Consolidate data security and validation, and better accessibility. Surgeons and nursing colleagues appreciated wound care illustration methodology. It is effective in engaging clinical handover with paperless documentation. The promising potential of paperless documentation is clearly revealed in health care system. Clinical album utilization should be embraced in clinical handover to mitigate paper documentation and polish clinical workflow as well as quality of care.
