Introduction :
In Cheshire Home, Chung Hom Kok, most of our patients are fragile elderly with multiple diagnosis, and sometimes their condition may deteriorate. Therefore, timely and efficient Cardiopulmonary resuscitation (CPR) is essential to provide survival opportunity to patients due to arduous accessibility of the nearest acute hospital from our remote hospital. Our staff have to be equipped to perform CPR well; and also the new equipment might be as powerful as expected. After comprehensive review, strategic efforts have been put into advancement of training and drills, equipment and communication.
Objectives :
1. To ensure the resuscitation quality in a remote hospital to enhance patient safety 2. To upgrade and increase resuscitation equipment to align with cluster hospitals 3. To strengthen the collaboration of involved department, particularly nursing and security guard through effective communication.
Methodology :
1. Training and Drill BLS Refresher Corner is set up for nurses to perform CPR and return demonstration twice yearly and coordinate with HKEC training center to conduct tailor-made simulation training on Emergency & Contingency Preparedness. The frequency of CPR Drill is increased from yearly to quarterly. Resuscitation equipment photo guides and training video are also developed to facilitate colleagues to review at any time. 2. Equipment The number of well-equipped crash carts is increased from 4 to 7 to provide full coverage in CCH, including all wards and residents’ activity areas. The advanced equipment including Lucas 3.0 compression system, video laryngoscope, and i-gel have been put in use. 3. Communication Due to limited manpower, particularly during night shift, the rapid communication is crucial. With the use of walkie-talkie, the emergency call process among wards and security department has been streamlined to strengthen the collaboration.
Result & Outcome :
1. ROSC Rate of Cardiac arrest (Non-DNACPR in-patient) No of CPR Successful ROSC Rate : 2021 60%, 2020 50% and 2019 0%. The overall ROSC rate was increasing from 0% in 2019 to 60% in 2021. 2. Staff Satisfactory Survey ● 100% staff agreed that through regular trainings and drills, they have more confidence in the resuscitation performance. ● 90% staff considered the introduction of new equipment helps facilitating the resuscitation process. ● 95% staff recognized walkie-talkie communication enable quicker emergent response from 5 minutes to within 3 minutes. Conclusion With systematic training, updated equipment, and effective communication, our staff are well-prepared for handling emergency situation. Survival rate of patient, thus, will hopefully be increased.
