Service Enhancement on peritoneal dialysis training program in renal unit of Caritas Medical Centre

Introduction :
A structured 5 days’ peritoneal dialysis (PD) training program to patient has been implemented in renal unit of Caritas Medical Centre for years.
Previously, patient training might be postponed due to patient condition change before PD training, the efficiency of training workflow was affected. Besides, different training material were used among nurse educators, so the quality of training might be varied. Hence, PD training was modified and a service enhancement program was launched since August 2021.
Objectives :
1. To improve the efficiency of training workflow.
2. To ensure the consistency of training information and skill will be delivered.
Methodology :
A new pre-training assessment checklist was used within 14 days after tenckhoff catheter insertion to early identify any further change of patient conditions such as impaired eye-hand coordination, cognitive and social problem. So, early intervention would be performed to tackle the problem before training.
New PD training material was designed, it consisted of PowerPoint, video (bag exchange procedure and tenckhoff catheter exit site care) and training poster. All training materials were standardized so that the consistency can be ensured and facilitated nurses teaching process.
Patient was required to assess the knowledge, trouble shooting and skill by advanced practice nurse before finishing the training. After passed the assessment, patient was awarded a graduation certificate for encouragement.
New patient training kit (pamphlet and bag exchange poster) and hotline will be provided to patient for supporting PD care in the community.
Patient and staff satisfaction survey were conducted after implementation of the program.
Result & Outcome :
From August 2021 to December 2021, 34 patients were attended PD training and 14 nurses as nurse educator.
All patients were trained as scheduled and the efficiency of training workflow was improved, as 5 patients (3 impaired eye-hand coordination, 1 cognitive problem and 1 social problem) were early detected and settled the problems before PD training.
The response rate of patient and staff satisfaction survey were 100%.
100% of patients (n=34) agreed that enhanced training program is suit their needs, they have confidence to perform PD treatment and disease management after the training.
100% of nurses (n=14) agreed that pre-training assessment checklist is helpful for early identify patient’s problem and make relevant referral before training.
All nurses agreed that the standardized training materials facilitate the teaching process.
The program will be assessed periodically to maintain the quality of service.
