Introduction :
Dementia patients developed behavioral and psychological symptoms are common during the hospitalization because of the disease progression, separation from family member or carers during hospitalization and which may lead to disorientation and restless. Patient psychologically face some stressful situations unmet needs, also the maladjustment to hospital environment may create uncertainty and confusion to patient with dementia
Non Pharmacological intervention managed dementia patient with BPSD can reduce the use of physical or chemical restrains on patient. It can also provide better care and decrease the severity or occurrence of BPSD in hospital
Objectives :
1 To reduce the use of chemical or physical restraints
2 To reduce the problems behavior such as aggression, shouting and wandering
3 To enhance the communication between healthcare providers and elderly
patients with dementia and behavioral problems
Methodology :
1.Assessment checklist to identify and recruit the target patients eg dementia with history of behavioral problems at home, suspected dementia with 5 mins MoCA score below the 2nd percentile. Patient in active delirium or confusion or psychiatric mental disorder eg schizophrenia, delusions were excluded in the program
2. Doll, music therapy and light therapy will be selected according to the individual intervention criteria
3. Information sheet to introduce the purpose and intervention of the program to the relatives
4. Record the patient behavioral problems in the behavioral record form to monitor the progress of the therapy
5. Provide the comfort and homelike environment in ward, environmental modification included the display sticker of potted plants, Chinese style calendar, home-like furniture and photo frame
Result & Outcome :
Total 13 cases were recruited in the program. Age between 84-97 years old Length of stay in hospital 13 -18 days
MoCA 2-12 score.
No patient was required the use of chemical restraint.
10 patients (76%) had be reduced the use of restraint, 2 patients ( 15%) were restraint free and 1 patient (9%) cannot reduce use of physical restraints
Established the positive relationships and increase the interaction between healthcare provider and patients with dementia
50% patients showed no behavioral problems, and 50% patients with mild behavioral problem, eg shouting and occasional insomnia
