Introduction :
Undergoing hospital renovation and relocation, WTSH infirmary wards were reorganized into a mixed gender infirmary (33 beds) and convalescent (15 beds) ward. Nursing staffs are facing challenge in care delivery approach. Long stay infirmary geriatric patients require long term care plan, and convalescent geriatric patients require discharge planning. Relatives are having different expectation throughout the whole caring process. Loss of trust, misunderstanding, lack of communication and conflict may arise and induce complaint if communication breakdown (Hogg, R., Hanley, J., & Smith, P., 2018). PRO were receiving multiple feedback and complaints from infirmary patient group. As such, formulation of coordinated care planning should involve good communication, trust building, and patient and carer satisfaction.

Objectives :
1. Strengthen the therapeutic relationship between nursing staff and patients/caregivers
2. Promote patients and caregivers involvement and collaboration in patient care plan
3. Improve patient care outcome
4. Minimize misunderstanding and conflicts
5. Improve service quality
Methodology :
A. Pre-program questionnaire of caregivers
To investigate and understand caring concern and difficulty of caregivers
B. Intervention by nursing team
1. Bedside or phone contact on updating patient condition with patient’s caregivers
i. Weekly (Infirmary patients)
ii. Day 0, Day 2 then every 4 days and prn (Convalescent patients)
2. Share infirmary and convalescent nursing experience
3. Peer group sharing activity
4. Proactive interview by supervisor
5. Explore expectation
6. Set up realistic goal
7. Face-to-face interview
8. Tea Party (11/1/2020)
9. Video Call (After outbreak of COVID 2019 on 25/1/2020)

C. Post-program satisfaction survey of nursing staff as outcome
Result & Outcome :
Pre-program questionnaire was conducted from 1-31 Dec 2019 for 17 groups of caregivers with response rate=94%.
Major caregivers’ worries during hospitalization involves condition deterioration, risk of developing pressure injury, slow wound healing process, hospital acquired infection and poor nutritional support.

39 infirmary cases with 368 times of relative update and 82 convalescent cases with 237 times of relative update were done via bedside and phone call from 1/1/2020 – 20/4/2020.
Main content of phone contact or interview involved:
1. Physical and mental condition
2. Appetite
3. Wound healing
4. Drug regime
5. Treatment plan
6. Psychological support
7. Tracheostomy care (Infirmary case)

Nil dissatisfaction or complaint received by PRO from relatives for post program follow up. Thankyou card and positive feedback were received from relatives.
Satisfaction survey of nursing staff (17 staffs) with response rate of 100% reflected satisfaction with this program.
