Introduction :
In Caritas Medical Centre (CMC), patients receiving coronary angiography (CORO) with/without percutaneous coronary intervention (PCI) will be clinically admitted to the Medicine & Geriatric (M&G) department for pre-and post-procedural care. In early 2019, a successful training program was launched to enhance nurses’ knowledge and competency in inpatient care with diagnostic CORO with/without PCI. However, the program had been suspended since 2020 in response to the coronavirus disease 2019 (COVID-19) pandemic while there were still cases necessarily admitted for the procedures. Furthermore, considering the high staff turnover rate in the M&G department and the complexity of the procedure, training for the new nurses would be an essential and challenging issue. Therefore, this program aims to develop a sustainable and effective training model specifying CORO and PCI case management for newcomers.
Objectives :
1. To recruit and train experienced ward nurses as “trainers” who will be responsible for the education and audit program for new nurses (trainees), specifying for CORO with/without PCI. 2. To equip the nurses with the knowledge and skills to competently care for a patient undergoing CORO with/without PCI.
Methodology :
Training materials and audit tools were prepared based on protocols released by HAHO, international guidelines, e.g., AHA, and previous training materials. The cardiac nurse provided a tutorial to four trainers to update the latest international guidelines regarding CORO and PCI and consolidate current standards and protocols for the procedure PCI in CMC. The assessment was performed after the tutorial to ensure 100% accuracy before proceeding to the training program for trainees. Trainers delivered the same tutorial under the supervision of the cardiac nurse to all (n=13) trainees. Trainees were audited before and immediately after the tutorial to assess their knowledge acquired. In addition, the compliance audit was performed through a one-to-one clinical supervision of trainees when the first time managing cases of CORO and PCI and reviewing their documentation simultaneously. Finally, trainees were requested to complete a survey to evaluate their self-competence.
Result & Outcome :
The service enhancement program was conducted from 11 October 2021 to 16 February 2022. All trainers (N=4) passed the assessment with full marks regarding the knowledge of CORO with/without PCI. The mean score of pre-and post-test as evaluation of trainees (N=13) increased from 74% to 89% after the tutorial given by trainers. All trainees achieved a 100% compliance rate in the practical audit of CORO and PCI case nursing management. In addition, all trainees (N=13) rated "agree" to "strongly agree" with their self-efficacy in preparing the patient for CORO and PCI and providing education to patients about puncture wound site management, whereas 92% (N=13) rated "agree" to "strongly agree" on managing post-procedural complications and providing post-operational and lifestyle modification advice to patients.
