A pilot study of self-management program in Occupational Therapy for patients with pain conditions in Collaborative Orthopedics and Family Medicine Service Model

Introduction :
A collaborative program between O&T and Family Medicine Specialist Clinic (FMSC) in managing SOPC service demand in KCC was implemented in OLMH in July 2020. Patients with pain conditions were referred to Occupational Therapy Department for early intervention on pain management and functional rehabilitation. Comprehensive self-management programs were established for patients with osteoarthritis knee (OA knee), lower back pain (LPB), neck pain and shoulder pain) and cumulative traumatic diseases(e.g. carpal tunnel syndrome (CTS), trigger finger (TF), tennis elbow (TE), and De Quervain’s tenosynovitis (DQV). Self management programs included patient education (symptom management, self management techniques in ADL and IADL tasks, energy conservation techniques, activities pacing, joint protection techniques and lifestyle redesign), practice of Health Qigong, functional rehabilitation and provision of splintage(corsets, knee brace and assistive devices) and ergonomic advice.
Objectives :
(1)To evaluate the effectiveness of self-management programs in Occupational Therapy
Methodology :
Subjects who completed OT program between October 2020 and September 2021 were included in this study. Outcome measures included:(1)Modified Barthel Index(MBI) and Lawton IADL(IADL) to measure functional performance,(2)Numeric Pain Rating Scale(NPRS) at rest and exertion,(3)Self administered questionnaires for measuring symptoms and functional performance including Knee injury and Osteoarthritis Outcome Score(KOOS), Oswestry Disability Index(ODI), Quick Disability of the Arm, shoulder and hand(Quick DASH). Outcome measures were administered at initial and final sessions for comparison and Wilcoxon Signed Rank Test was used for statistical analysis.
Result & Outcome :
142 patients completed the whole program. There were 62% females. Their mean age was 62. The mean number of sessions attended was 6.5. Most of the patients were having OA knee(53%) and LBP(44%), other were neck pain(14.8%) and CTD(11.2%). There was significant difference in MBI and IADL for OA knee, LBP but no difference for neck pain and upper limb pain conditions. There was significant difference on pain relieving in all subjects with all pain conditions. For symptoms and functional performance, there was significant difference in KOOS for OA knee, ODI for LBP, DASH for upper limb pain conditions but no difference for neck pain. Conclusions: Self-management programs in Occupational Therapy were effective to relieve pain for patients with different pain conditions.
