Use of communication slip to facilitate intra-ward bed transfer among nurses, Patient care assistants (PCA) and clerks, in order to minimize errors associated with patients transfer and increase staff satisfaction.

Introduction :
In view of the threat of the COVID-19, an admission cubicle was introduced during the summit period of COVID-19. Newly admitted patients were arranged in the same cubicle. They would be allocated to other cubicles after they had a negative COVID-19 screening result. The frequency of bed transfers thence significantly surged. There were chances of having multiple bed transfers simultaneously. Bed transfers would be initiated by nurse in-charge but executed by patient care assistants and clerks or team nurses.
Objectives :
To avoid communication breakdown which might hinder the transfer process or even result in wrong patient swabbing in serious cases, a communication form was introduced to enhance communication among the three parties.
Methodology :
A communication form was introduced to enhance communication among the three parties including patient care assistants, team nurses and clerks.


1. Patient care Assistants
Patient care assistants were responsible for transferring the patients and their belongings, and updating the communication chart for measuring temperature and foley drainage. They would have a clear instruction of the sequence for bed transfer for corresponding patients.

2. Team Nurse
Team nurses were responsible for acknowledging patients about the transfer, and managing patients’ medication and their booking for ECG and blood taking.

3. Clerks
Clerks would help exchange patients’ clinical notes and the CMS, and update the communication whiteboard and the diet order report.
Result & Outcome :
Intra-ward bed transfer can be carried out smoothly without communication breakdown. No transfer error about patient identity and patients’ belongings were reported. Out of 30 colleagues interviewed, 23 colleagues graded the communication slip with satisfaction. staff satisfaction survey by google form
