Introduction :
More than 34% of Hong Kong people are suffered from chronic pain(1) and significant number of patient suffered from severe psychosocial impairment. It may not be sufficiently managed in standard care. MAP program is mindfulness based multi-disciplinary cognitive behavioral program specific designed by pain team KWH for this subset of Hong Kong Chinese Chronic pain patient.
Objectives :
To review the long term (1 year) clinical outcome of chronic pain patient after MAP program
Methodology :
Prospective, Pre and Post-1-year clinical outcome analysis of subject who completed the MAP program between 2015 – 2019
Result & Outcome :
A total of 20 participants complete the MAP. M: F (2:18) & Aged from 35-76 15 out of 20 completed the Pre and Post-1-year psychological test. The statistical significant result p<0.05 as shown below: Psychological test Pre Post-1-year, The Highest Pain Level 8.60 to 5.8; The Lowest Pain Level 4.27 to 3.27; The Average Pain Level 6.40 to 5.47; Work and Social Adjustment Scale (WSAS) Home Management 5.73 to 4.07; Social Leisure 5.73 to 3.33; Private Leisure 5.13 to 2.93; Total 23.33 to 15.40; Depression Anxiety Stress Scale-21 (DASS 21) Depression 20.67 to 13.87; SF-12v2 (Role- Physical -2.16 to -0.93; Bodily Pain -1.98 to -1.33; Vitality -1.18 to -0.59; Role – Emotional -1.85 to -0.73; Mental Health -1.31 to -0.15; Physical Component Summary -2.22 to -1.54; Mental Component Summary -1.31 to -0.11; Chinese version of the Pain Self-Efficacy Questionnaire (HK-PSEQ) 40.80 to 54.67; Chinese Pain Catastrophizing Scale (HK-PCS) Rumination 10.53 to 7.73; Magnification 6.20 to 4.13; Helplessness 14.67 to 10.33; Total 31.40 to 22.20; 9 out of 20 participant completed the Pre and Post-1-year physical fitness test. The statistical significant result p<0.05 as shown below: Physical fitness test Pre and Post-1-year, The stand tolerance 31.88mins to 48.5mins; The Six minutes walk test (6MWT) 343.04m to 419.79m 20meter walk test 19.11s to 15.33s; 40m Weight bearing test 7.68lb to 10.6lb; 2min stair steps test 89.93 to 127.38; Arm Curl Test 15.73 to 21.25; It is the first report on long term outcome (Post-1-year) of chronic pain patient who have completed the MAP program between 2015-19. The preliminary data shown significant and sustained improvement in multiple physical and psychosocial outcomes after finishing the MAP program including better pain score, better physical fitness, improvement in quality of life, less depression, improved self efficacy, less catastrophizing, and better work and social life.
