Introduction :
In medication safety, nurses play a unique role and responsibility that they are the final person who check the medication if it is correctly prescribed and dispensed before administration.
There were five medication incidents reported between 2020 and 2021 in the Orthopaedics department. This rising number of incidents raised an alarm that action is needed to prevent further incidents happening. New comers are usually unfamiliar with the ward practice that prone to medication incidents occur. The last CQI program was held four years ago. In order to minimize the rate of medication incident, a CQI program was carried out in 2021.
Objectives :
1. To minimize the rate of medication incident.
2. To strengthen nurses’ knowledge on medication safety.
3. To enhance nursing staff drug administration process and communication by introducing practical ways in checking drug and handing over to create safe medication culture.
Methodology :
This program was carried out by reviewing the medication incidents retrospectively. During the reviewing process, various learning points were identified out and shared among all the nurses in the department by holding sharing sections. In addition, a poster was designed with "five rights" as the main theme, so as to remind nurses the elements during drug administration. Besides, the medication safety guidelines of Hospital Authority were reviewed.
Result & Outcome :
Spot check on administration of medication (AOM) audit will be done to evaluate staff knowledge and performance on AOM.
The prevalence rate of medication incident will be evaluated at the end of 2022.
