Enhance Cognitive and Activity of Daily Living Performance of patients with Fragility Hip Fracture

Introduction :
Functional and cognitive decline is common during hospitalization, particularly in elderly. During the COVID-19 pandemic, all visiting arrangement in Hospital Authority are suspended. A structured intensive Cognitive and ADL training has been started to increase cognitive stimulation and participation in functional tasks during hospital stay.
Objectives :
To evaluate if a structured ADL and cognitive training for fragility hip fracture cases could minimize functional and cognitive decline during hospitalization
Methodology :
All cases recruited in Fracture Hip Pathway during June 2019 to Jan 2021 are captured. Their compliance rate in ADL training, cognitive profile in Abbreviated Mental Test (AMT), subscore in year, date, time, place and person (reality orientation) are analyzed.
Result & Outcome :
A total of 333 cases (256 females, 77 males) are captured, mean age 85 ± 7.6 (65-103). Average number of training sessions is 4 during this acute hospital stay. The overall compliance in ADL training is 99%, with 86% independent in grooming, 11% required assistance, and 2% required supervision. To compare the change in cognitive performance, Paired T-test shows significant improvement in reality orientation, t (327) = 4.30, p<0.001. Cases are further analyzed in three groups according to the level of cognitive performance in AMT, i.e. AMT =0, 1-6, and >=7. Patients with AMT score 1-6 shows significant improvement compare to the other two groups. Conclusion: A structured ADL and cognitive training for fragility hip fracture cases could minimize functional and cognitive decline during hospitalization.
