Introduction :
Patient discharge document zip bag with QR code of “HA Go” application.
Objectives :
1.Elderly patients can handle discharge documents tidily upon discharge. 2.Promote patients / their family members to use “HA Go” as tool to arrange and remind their medical appointments systematically.
Methodology :
1. Engage the nursing colleagues, ward supervisors during the planning stage. 2. Discuss and make the plan with working group members. 3. Briefed the TBCU colleagues about the proposed use of patient discharge document zip bag to handle discharged documents for elderly patients upon discharge. 4. Consult staff and supervisors' comments to refine the plan 5. Implement the project in TBCU wards after briefing sessions to nursing colleagues 6. Evaluate and debrief the successful result with colleagues
Result & Outcome :
Enhance the quality of service on patient discharge 1. 45 elderly patients / family members who had received the patient document zip bag in the first month of the project (pilot phase). They had also been introduced to install the "HA Go" application upon patients discharged. 2. Results of staff satisfaction survey had shown that the discharge document zip bag was useful for elderly patients to handle discharge documents properly and the QR code helped patients and their family members to recognize the function of “HA Go” application.
