Introduction :
IV access for IV chemotherapy, IV infusion or IV medication is very common in Oncology ward. Oncology patients sometimes may have more than one IV access for multiple use. Difficult venous access and fragile in skin condition to have IV access are known common problem. As an oncology nurse should pay awareness and observation on IV access in daily nursing care. Proper documentation is one of the important and effective way to maintain the standard in nursing care.
Objectives :
1. To prepare and standardize the format in documentation of IV access site condition.
2. To enhance nursing staff awareness by using designated stamp chop as routine assessment in every shift.
3. To provide a quick guide for the junior nursing staff how to observe and document IV access in proper format.
Methodology :
1. To observe and collect the data in documentation of IV access before using the standardize stamp chop.
2. To collect the nurses’ opinions for design the stamp chop and ordered the items.
3. To conduct briefing to nurses for trial use the stamp chop in document of IV access site in Integrated Progress Notes.
4. To assess the IV access site location and any abnormalities (such as infection) by use the stamp chop.
5. To review the compliance and effectiveness of using the stamp chop periodically after implementation.
6. To collect the feedback from nursing staff for the enhancement.
Result & Outcome :
Evaluation was conducted to collect the feedback from nursing staff in using the stamp chop after two months. The majority of nursing staff agreed this practice can enhance the nursing practice to standardize the format in documentation of IV access site condition. The stamp chop provides a guide for the junior nursing staff to observe and document IV access. The stamp chop also enhances nursing staff awareness of IV access site condition.
