Introduction :
Responding to increasing complexity of wound care across different wound types due to aging population and multiple chronic diseases, as well as high volume of wound care needs in community settings, appropriate level of training for frontline nurses to enhance professional knowledge and skills are of importance to assure quality of care. Based on the 3-Tier Service Modecl for Wouind Care Service in KCC (KCC Nursing Service Coordinating Committee in Wound & Stoma Care 2022), individualized training needs of frontline nurses were supported to attend various levels wound training to enhance staff learning and professional development.
Objectives :
To cultivate learning culture among nurses in KCC GOPCs To promote and enhance the wound care knowledge or skills transfer To promote evidence based practice in wound care service in primary health care settings To ensure continuity and quality care for patients receiving wound care service
Methodology :
Appropriate levels of wound training is provided for frontline nurses based on their competencies and service needs. For all qualified nurses especially newly joined nurses (1st tier) who provide management of simple wound, linear wound and uncomplicated pressure injury, basic wound training is offered to enhance nurses’ clinical knowledge on wound healing process, and basic wound care on difference wound types etc. For wound link nurses and those serving in wound clinics of FM&GOPC (2nd tier) and providing advanced wound management, intensive training (i.e. Enahncement Program on Wound Management (organized by QEH) is offered. Clinical practicum is also included in the training to facilitate nurses’ application of theory into practice. Recognized 3rd tier wound nurses holding qualification in advanced wound care (e.g. PRCC in stoma and Wound Care Nursing) serve as wound team coordinators and clinical mentors to provide guidance and assessment during the clinical practicum and daily practice. Meanwhile, regular departmental wound case conference is organized to facilitate knowledge and expereince sharing, as well as cultivating learning and teaching culture.
Result & Outcome :
Front-line nurses including APN, RN and EN (n=102) are targeted to receive appropriate level of wound training. 1st tier nurses (n=87, 85.2%), 2nd tier nurses (n=23, 22.5%), and 3rd tier nurses (n=2, 2.0%) have received appropriate level of wound training. Also, 10 wound case conferences including 44 topics have been organized from 2017 to 2021, of which 29 speakers were invited and 209 nurses attended. Both speakers and attendants had positive feedback of increasing wound knowledge, good clinical practice, practical and presentation skill, and passion in wound care. Wound care service quality could be assured by our structural wound training program for nurses. Learning culture could be cultivated among nurses by providing different levels of wound trainings according to their competencies and service needs. Learning and sharing of wound management among nurses working in GOPC wound service could promote team spirit and professional development.
