Introduction :
Proper puff technique and breathing exercises for all patients with Chronic Obstructive Pulmonary Disease (COPD) were significant in symptoms control and disease management. However, the conventional face-to-face education and COPD classes using leaflets / booklets were suspended in all General Out-patient Clinics (GOPCs) in the New Territories West Cluster (NTWC) during COVID-19 pandemic. Therefore, a Continuous Quality Improvement (CQI) Project aiming at enhancing the teaching models of puff technique and breathing exercises by using recorded video clips (accessible by QR code) was implemented.
Objectives :
1. To educate and empower COPD patients on proper puff technique and breathing exercises efficiently, effectively and confidently
2. To minimize the risk of cross infection during patient education
3. To standardize the teaching materials in NTWC GOPCs on proper puff technique and breathing exercises
Methodology :
The CQI project was initiated from January, 2021. It could be divided into 3 phases. The first phase was started from January to April, 2021, targeted on the preparation and production of the teaching video.
The second phase was a pilot study, conducted from May to June, 2021, aimed at the evaluation of the effectiveness for the new teaching models on puff technique and breathing exercises. Total 20 patients were recruited in the pilot study. Patients’ performance of puff usage and breathing exercises were assessed and evaluated by an identical questionnaire before and after watching the video.
The final phase was the implementation phase which has been rolled out since July, 2021.
Result & Outcome :
The result of the pilot study showed significant improvement on the new teaching models of puff technique and breathing exercises for COPD patients. Data collection for outcome of the implementation phase is ongoing.
