Introduction :
The Inpatient Patient Follow-up to Frist Attendance Ratio of Shatin Hospital in the past few were below the HA Dietetics services overall standard. As one of the major extended care hospitals in NTEC, accessibility and continuity of nutrition care are essential elements for facilitating the rehabilitation journey of our patients.
Objectives :
To improve the Inpatient Patient Follow-up to Frist Attendance Ratio by enhancing workflow and redesigning staff roles and responsibilities.
Methodology :
1) Deliver value - Redesign Dietitian and PCA roles in inpatient service to focus more on improving effectiveness, efficiency and timeliness of patient care. • Dietitian i/c - redistribute clinical and administrative works proportion. • Dietitian - reinforced reasonable first to subsequence intervention ratio at an extended care hospital. • PAC - relocated the duty focus to patient care rather than clerical works. (assisting dietitian to collect findings for new case assessment and subsequence care management) 2) Continue improvement - Revamp the department's inpatient subsequent intervention mechanism and clinical pathway to triage care according to nutritional needs. • An inpatient subsequence care management system was in place to focus on unstable, high malnutrition risk, and outcome-driven cases instead of using a board and undefined criteria to allocate resources 3) Eliminate waste – Improve the working process using an electronic patient record system to reduce repetitive tasks.
Result & Outcome :
1) Follow-up to First Attended Ratio improved by 30% in 2020-21. Follow-up to First Attended Ratio in 2019-20 = 0.6, whereas Follow-up to First Attended Ratio in 2020-21 = 0.78 2) Increment in follow up intervention and total attendance. Follow up attendance from 2019-20 (2975) to 2020-21 (4002) increased by 35%. First attendance from 2019-20 (4941) to 2020-21 (5159) increased by 4.4%. Total attendance from 2019-20 (7989) to 2020-21 (9161) increased by 16% (with adjusted monthly average FTE change was 2.2% from 2.36 to 2.41 FTE).
