Introduction :
Retained materials after surgery is a Sentinel Event (SE). According to past incident reports in HA, retention of dressing material was one of the most common incidents related to wound care. Any retained dressing material could be detrimental to patient safety and delay wound healing. On the other hand, using appropriate dressing material is one of the key factors to promote wound healing. However, many nurses lack the knowledge to choose the optimal dressing product during wound care. Therefore, we implement this program to improve the quality and safety of wound management, as well as increase staff knowledge and competence in dressing selection.
Objectives :
1. To minimize the risk of dressing material retention in wound packing. 2. To standardize the documentation on wound dressing. 3. To empower the staff’s knowledge on the selection of dressing materials for effective wound healing. 4. To examine the effectiveness of quality improvement on wound care management for O&T patients.
Methodology :
1. Use alert signage on “E-bed panel” to increase staff awareness on wound packing. 2. Apply a label on the outer dressing to alert the packing details of that wound. 3. Use “Wound Dressing Chop” to ensure clear and standardized wound dressing documentation. 4. Design a quick reference clinical guide for the application and selection of wound dressing materials.
Result & Outcome :
Enhance the wound care management by minimizing the incident rate of dressing material retention and empowering the staff's knowledge on the selection of dressing materials. 1. No more incident on the retention of dressing material. 2. Positive feedback and satisfaction from nursing staff. 3. Increase in nursing compliance rate of the interventions.
