Introduction :
Most residential care homes (RCHEs) in HKWC do not have or only have one computer with CMS Citrix available. In HKWC, usually 2 to 3 doctors would go to RCHE to see patients in CGAT clinic each session. Hence, one computer in RCHE is not enough to support the CMS use. Moreover, the internet network of many RCHEs is quite unstable. Therefore, many CGAT doctors had to handwrite the consultation notes and medication prescriptions at CGAT clinics in the past. This might increase medication incidents and errors. Not only this, patient privacy issues were also concerned as doctors needed to bring back medication prescription sheets to hospital pharmacy after each CGAT clinics.
Objectives :
To achieve 100% coverage using electronic drug prescription and progress notes via remote CMS during CGAT clinic by BYOC program
Methodology :
A working group was formed in 2020. Work flows was then developed and endorsed. Computer notebooks and printers were purchased. New fax line, new fax machine and new CMS station were installed. Pilot implementation with 15% of HKWC RCHE was performed in 2021, with gradual increment of the number of RCHE implementing the new workflow.
Result & Outcome :
BYOC has been fully implemented in HKW CGAT clinic smoothly in 1Q 2022. There is now 100% coverage using electronic drug prescriptions and progress notes via remote CMS during CGAT clinic. All parties including doctors, nurses, pharmacists and RCHE staffs welcome and are confident to work under the BYOC program. The medication errors at CGAT clinics are significantly reduced and patient’s data privacy is enhanced.
