Transitional care program for grown-up Venitlator-assisted Individuals (VAI) at DKCH

Introduction :
Grown up VAI patients in DKCH were previously solely managed by paediatric team. However, multiple difficulties faced. When the patients deteriorated and required to transfer back to QMH for management, they were escorted by DKCH Paediatric Medical officer to QMH A&E. Then the patient needed to wait at A&E to be triaged to adult medical ward or AICU. Moreover, difficulties were also faced when arranging QMH medical SOPC FU or in-patient consultation for these patients.
Objectives :
To establish the Transitional Joint Care program for these grown-up Ventilator-assisted Individuals in DKCH.
Methodology :
A working group for this Transitional Joint Care program was formed in early 2022. In initial phase, transitional joint care would be provided by medical team and paediatric for 5 long stay grown-up VAI in DKCH which has already been started in 7/2022. In later phase, community-dwelling VAIs clinically admitted will also be recruited to the transitional care. Joint Paed-Med clinic for VAI with transition to Adult VAI clinic would also be established also in later phase.
Result & Outcome :
Work flow of daily ward round and weekly grand round was established. The resuscitation workflow was confirmed and endorsed by resuscitation committee. Once patient deteriorate, both medical on call doctor in FYKH and pediatric doctor in DKCH would both engage to resuscitate the patient. The logistics of transferal of these VAI once deteriorate was also established. These patients can be transferred to QMH adult AICU/medical ward without going to QMH A&E if deem necessary. 5 VAI were managed under the Joint care program uneventfully. The doctors, ward nurses, allied health colleagues, patients and relatives all welcomed the new program.
