Introduction :
There is an increased environmental contamination rate of CRA and MDRA in wards from 24% to

37.5% in 2Q 2022 in LKB. In order to increase the staff awareness of infection control and reduce the

Hospital Acquired Infection rate, a “Six Nursing procedure program” which included shower bathing,

napkin round, wound dressing, enteral feeding, open suction and urine bag emptying was

implemented in all wards in LKB since July 2022.
Objectives :
To reduce the infection rate through enhanced basic infection control practice:

1. Hand hygiene

2. Enhanced infection control measures in six nursing procedures identified.

To reduce the working environment contamination of MDRO
Methodology :
Methodology

In order to control HAI MDRO infection in LKB, various preventive measures were taken. They were

mainly referred to three aspects:

1. Six Nursing Procedures

1.1 To form a workgroup and align the practice of six nursing procedures

1.2 To standardize the infection control measurement of the six basic nursing

procedures in LKB.

1.3 To promulgate the standard of practice to ward staff

1.4 To conduct cross ward audit to the wards staff in LKB

2. Hand Hygiene:

Ward Infection Control patrol nurse were identified to monitor staff hand hygiene

compliance and promote alertness on the importance of hand hygiene.



3. Environmental Cleansing:

3.1 Twice per day cleansing of bed table, bedside rails, call bell of all beds Oxygen

flowmeters & Suction flowmeters, monitoring and reporting of malpractice on the

disinfection procedure to cleansing team supervisor;

3.2 using designated items for MDRO infected patients such as designated eVital

machines, stethoscope and water bottle;

3.3 using of disposable BP cuff and disposable oximeter finger probe for MDRO patients

from admission till discharge.

3.4 Perform regular ward environmental cleansing and cleansing of common shared

used items by ward PCA / agency PCA three times per day.

3.5 Perform weekly disinfection of common shared used items by UVDI 360
Result & Outcome :
Four wards in LKB were passed in cross ward audit of shower bathing & enteral feeding and

the overall compliance rate was over 96%.

LKB Staff hand hygiene audit compliance rate in 3Q 2022 was 93.56 % (PMH staff compliance

rate 87.00%)

In comparison to 3Q with high bed occupancy in 2021 & 2022, there was around 11%

decreased in newly hospital acquired MDRO infection.

In year 1Q to 3Q 2022, there was a significant decreased of MDRO environmental

contamination rate from 23.49% to 6.17%. Candida Auris environment contamination rate

was decreased from 7.7% to 0% from 2Q 2022 to 3Q 2022.

The actions taken in the program were significantly effective in the prevention of MDRO

transmission and reduction in HAI MDRO infection.
