Introduction :
High flow oxygen (HFO) is a non-invasive respiratory support and proven to have clinical and physiological benefits including high oxygenation, alveolar recruitment, humidification and heating, increased secretion clearance, reduction of dead space and can prevent endotracheal intubation. During the 5th wave of COVID-19 with dramatically worsen situation in Hong Kong, there were many critically ill patients who developed acute hypoxaemic respiratory failure and required HFO therapy which was relatively new for extensive use in HAHO. A series of training and actions had been implemented to enhance competency of nursing colleagues to care patients with HFO therapy in KEC during the 5th wave of COVID-19 in KEC.
Objectives :
To enhance competency for caring COVID-19 patients in Kowloon East Cluster
To evaluate the knowledge improvement after the train-the-trainer courses
Methodology :
A series of training sessions and measures had been implemented to enhance colleagues’ competency on caring patients receiving HFO therapy as follows:
• One cluster-based open forum in March 2022
• Eight identical Train-the-trainer (TTT) sessions from May to July 2022
• A competency checklist
• e-learning with one CNE point granted which had been developed by Specialty Advisory Group (Respiratory), HAHO
• Three identical lectures with workshops from March to May 2022
• Specialty Nursing Practice Guideline on High Flow Nasal Cannula was coordinated and developed by Nursing Service Division of HAHO in the end of 2022.
• A numbers of onsite training sessions
Result & Outcome :
Results
Total eight identical 2-hours TTT workshops had been conducted with 2 CNE (continuing nursing education) points granted in June & July 2022. There were 169 participants of nursing colleagues ranked from RN to WM from different Departments of three hospitals in KEC. Their knowledge of caring patients with HFO therapy was improved by 52% from mean score of 5 to 7.6 over 10 via pre and post workshop quiz. Satisfaction survey was conducted with overall mean score of 5.44 over 6.
Besides, a cluster-based forum with more than one hundred participants. Besides, three identical 1.5 hours of lecture and skill workshop for 70 participants had been implemented from March to May 2022. Moreover, a competency checklist of and e-learning of caring patients with HFO had been developed.

Conclusion
A series of training sessions had been taken to enhance competency of nursing colleagues to care patients with HFO therapy in KEC during the 5th wave of COVID-19.
