Introduction :
AHNH & TPH has a unique system for reporting pressure injury (PI) over the past years. However, the reporting process and format are complicated to obtain accurate and traceable PI situation. It was not effective for nurse managers to grasp the information at golden period to prevent patient from PI suffer.
Objectives :
To automate the reporting system with mistake-proof capability which smoothen the workflow and enhance accuracy and efficiency. Developing an electronic situation dashboard on PI instance fosters and facilitates nurse manager to tackle problem right ahead. This also provides information on pressure injury tracking at multiple episodes so that the nursing care can be reflected effectively.
Methodology :
Nursing and clerical staff of Central Nursing Division(CND) of AHNH & TPH collect data, validate and generate reports. During the process of data validation and analysis, time spend on various workflows were the key workload consumption. A user friendly database program is designed to reduce redundant works (target >50% reduction in workflow). It aligned the pressure injury incident reporting machanism through creating a structured interface to collect, analyse and generate reports with just a few clicks. An application interface with mistake-proof function helped CND’s staff to input data efficiently and correctly. The works of data analysis and report generation will then be done by the system. A concurrent update on Pressure Injury Dashboard can provide updated situation for nurse managers to plan and facilitate prevention and rectification of pressure injury induced care management.
Result & Outcome :
90 percent reduction of time in report generation was achieved. Various format of pressure injury reports will be sent automatically to local nurse managers as well as cluster’s CND with just one click of “Generate & Email” button. An efficient and accurate situation dashboard on pressure injury instance will be updated by just one click of “Export to dashboard” button. This dashboard on iCND facilitate access from nurses on current situation, information includes the number of incident, source of pressure injury and their severity. It provides instant picture from hospital to ward level that nurse manager will be able to grasp the situation easily and plan for prevention and enhancement. Electronic template for ward to report Pressure Injury is a way forward to further enhance efficiency and safe the environment through reduction on paper use. It was developed and under testing for the second phase of implementation plan.
