Introduction :
Fall is one of the commonest reasons for admission for elder patients. A highly-structured Occupational Therapy program is devised to incorporate home assessment, fall prevention education and home-based rehabilitation training for the post-discharged ICM patients with recent fall history. It aims at 5 sessions over 8 weeks. Participants’ ADL and IADL performance, fall efficacy, fall prevention knowledge, and satisfaction toward this service will be evaluated.
Objectives :
1. Develop a checklist for comprehensive home assessment 2. Equip patients or caregivers with fall prevention knowledge by on-site education, supplemented by educational pamphlets or HA Go app 3. Provide structured home rehabilitation program (e.g. Health Qigong) to improve ADL and IADL performance
Methodology :
1. A home-risk checklist was developed to assess the potential fall hazards, and guide therapists to advise home modification 2. A structured fall rehabilitation content was developed to reduce recurrent fall at home or in community. The content includes Health QiGong, LiFE home-based training and fall prevention education leaflets.
Result & Outcome :
Since Jan 2022, 6 ICM patients were recruited. Preliminary results indicated that all showed improvement in ADL and Fall efficacy. All subjects had positive feedback to the program. They agreed that the fall prevention knowledge was useful and home program was beneficial to their balance, limbs coordination. However, some patients declined further home visit due to outbreak of COVID19, thus this study needs further case recruitment and data analysis.
